Ways to Connect with Patients Who Have a Rising Body Mass Index

ABOUT
Patients with elevated BMIs and a diagnosis of overweight or obesity often need support to assist with establishing healthy behaviors that extend beyond the patient-provider interactions. Connections with community resources and technological devices are examples of the support options available to patients in health centers.
This webinar will provide information on how health centers coordinate care with community food resources and community wellness centers. RJHA will discuss the use of technology and devices to help patients self-manage their weight loss journey. These support services and devices are options for health centers to include in their BMI follow-up plans. This webinar aligns with the Health Center Performance Improvement domain of Quality, Patient Care, and Safety as it addresses how to use technology and community organizations to improve health outcomes associated with an elevated BMI. This training will be in conjunction with HealthLeads.
PRESENTERS
Director, Clinical Services
Christina Mister has over 13 years of experience as a nurse leader in community, hospital, health insurance, and regulatory settings.
Christina has had a valuable impact on healthcare quality improvement. She has had positions in nursing leadership, nursing management, case management, payer quality programs, program management, regulatory compliance, and nursing education.
Most recently, as a Centers for Medicare & Medicaid Services (CMS) nurse consultant, Christina has provided accreditation oversight to Medicare’s acute care programs. She serves as the program lead for Diabetes Self-Management Training and Home Infusion Therapy accreditation programs. Additionally, she is the clinical support for Rural Health and Federally Qualified Health Center programs. Christina provides oversight to national accreditation organizations and provides direction in the development of education material for state surveyors for those programs. Additionally, she is responsible for collaborating with other federal agencies and national associations.
As a program manager for Johns Hopkins Healthcare, LLC, Christina aligned measurable program goals with the organization’s mission and utilized quality improvement strategies and measurement to implement changes in the health plan’s programs. Christina has been instrumental in improving the health of the community; as a Program Manager, she collaborated with external stakeholders to improve health outcomes and decrease the high utilization of members within the Complex Case Management and Disease Management Program.
As an FQHC Clinical Manager, she has vast experience in health center operations, care management, health center budgeting, nursing management, and medical assistant supervision.
Christina has a Bachelor of Science in Nursing from Coppin State University (Baltimore, Maryland) and a Master of Science in Nursing Administration from Morgan State University (Baltimore, Maryland).
Cynthia Vuittonet,
MD, FACP, FASAM
Chief Medical Officer, Central Jersey Medical Center
Dr. Cynthia Vuittonet is triple board certified in Addiction, Internal, and Obesity Medicine. She received her undergraduate degree in Psychology from Brown University and her medical degree from the Warren Alpert Medical School of Brown University. She completed her internal medicine residency at the University of Massachusetts. Dr. Vuittonet is dedicated to helping patients receive the treatment they need so they can enjoy a positive and fulfilling lifestyle. Fluent in both English and Spanish, Dr. Vuittonet is uncompromising in her dedication to the well-being of her patients and strives to provide the best care. Dr. Vuittonet has been published in many peer-reviewed journals and is the recipient of many prestigious awards, including the American College of Physicians Young Achiever Award. Due to her unwavering dedication to the field of medicine and delivering the best care possible to her patients, Dr. Vuittonet was inducted as a Fellow in the American College of Physicians and Fellow in the American Society of Addiction Medicine. She is committed to providing skilled, comprehensive health care in a warm environment and seeks to build strong, long-lasting relationships to optimize patient care, education, and outcomes. She is passionate about caring for patients with various addictions and helping them achieve recovery and lead a fulfilling life. Dr. Vuittonet is currently the Chief Medical Officer at Central Jersey Medical Center, a Federally Qualified Health Center, and an Assistant Professor of Medicine at Hackensack Meridian School of Medicine.
Dominique Morgan,
MPH
Chief Operating Officer, HealthLeads
Dominique Morgan has decades of experience and a passion for transforming health outcomes for populations. As the chief operating officer, she oversees Health Leads programs, initiatives, finance and business operations all with an aim at driving change to improve health outcomes.
Prior to joining Health Leads, Dominique was the market president for VillageMD of Michigan and founder of Morgan Solomon Consulting, a healthcare consulting practice, focused on population health and value-based care solutions. She also served as chief operating officer of Steward Medical Group and vice president of population health for Steward Health Care and was with Kaiser Permanente in Northern California for eight years leading Northern California complex needs populations portfolio and women’s health.
As a national speaker on accountable care organizations (ACOs), Medicaid expansion, and community collaboration, Dominique frequently draws on her experience to assist other care delivery systems across the country, helping develop and enhance their care management and population health efforts.
Dominique currently serves on the National Board of Peer Health Education and Franciscan’s Children’s Hospital in Boston.
Dominique received bachelor of arts degrees from Cornell University in biology and political science, and a master of public health in health policy from the University of Michigan. She is also a member of the American College of Healthcare Executive (ACHE), National Association of Health Service Executives (NAHSE), and American Public Health Association (APHA). Dominique most recently was awarded the 2024 Women of Excellence award by the Michigan Chronicle. Dominique resides in Detroit, Michigan with her two teenage children.
The AAFP has reviewed Preventive Services and Chronic Disease Strategies for Health Centers and deemed it acceptable for AAFP credit. The term of approval is for one year. Participants should claim only the credit commensurate with the extent of their participation in the activity.
Each live session is approved for 1.0 AAFP Elective credits. Elective credits are available for physicians, physician assistants, nurses, nurse practitioners, and medical assistants.
